Horse Donation Form The following horse is to be donated to The Headshaking Syndrome Center and Pam Neff for the purpose of studies related to headshaking syndrome in horses. This transfer will include full ownership of the horse to The Headshaking Syndrome Center and Pam Neff and the previous owner will no longer have any rights to the horse. The previous owner has made sure that there are no unattended to liens or debts related to the horse. The horse must be otherwise healthy and not have any of the following conditions: Infections Immunosuppressive conditions such as pituitary pars intermedia dysfunction
